This innovative program is available to both full-time and part-time
associates who work a minimum of 32 hours per week. As a member of our
Pay Per Visit Program, you are:
- Paid Per Visit as well as Paid Weekly
- Eligible for Comprehensive Benefits that include Medical, Dental,
Vision, company matching 401k and stock purchase plan
- Free from the typical home care administrative paperwork as you will
not complete expense reports or time studies
- Paid mileage from Visit Notes and Mileage is paid weekly
- Eligible for Paid Time Off

dysfunction: Gentiva Safe Strides. Led by physical therapists, this
unique program offers a one-of-a-kind career challenge, specialized
training and career advancement opportunities.
As a Pay Per Visit Safe Strides Physical Therapist with Gentiva, you
will:
- Test/screen the patient's physical strengths to assist the physician in
evaluating the patient's level of function and record findings to
develop or pursue treatment programs and establish measurable training
objectives.
- Develop/implement a conditioning/rehabilitation program consistent with
physician's Plan of Treatment and the overall goals of the patient/rehab
team. Adjust treatment as needed to achieve maximum results.
- Confer with physician and other team members to obtain additional
patient information and assist in developing, implementing, and revising
the therapy treatment program and Plan of Treatment.
- Integrate physical therapy with other aspects of patient's healthcare
and training needs in accordance with physician's orders. Participate in
family education meetings.
- Coordinate, oversee, supervise, instruct, and evaluate Physical Therapy
Assistants' performance in implementing physical therapy services.
Train Physical Therapy Assistants to implement treatment protocols and
collect data on patient progress as permitted by policy and regulation.
- Act as a liaison and maintain contact with patients, physicians,
Clinical Manager(s) and other members of the healthcare team and the
community in assuring effective care coordination and appropriate
resource referrals.
Qualifications
- Degree from an accredited Physical Therapy Program (approved by the
American Physical Therapy Association, the Committee on Allied Health
Education and Accreditation of the American Medical Association and/or
the American Physical Therapy Association) and meet any additional
qualifications as stated in state homecare regulations
- Six months of experience
- Current licensure/certification in the state of practice and current
CPR certification
- Good organizational and communications skills
- Ability to function with minimal supervision
